Type locality. Perú, Loreto, Pacaya–Samiria National Reserve, Pacaya River (5º16ʹ28ʹ S, 74º29ʹ55ʹ W). Holotype: MLP 5431 (whole–mounted specimen). Paratypes: MLP 5432 (2 whole–mounted specimens), MLP 5433 (2 sagitally sectioned specimens), MLP 5434 (1 whole–mounted specimen).
