Redescription. Forewings 2.2 times longer than wide, length 4.2–4.5 mm. Colour stramineous; facial carinae and middle of pronotum light orange-red. Forewings white, lightly powdered; four brown-black marks around apical cross-veins between Ms1b, Ms1a, Ms2, Ms3 and Ms4; a tiny dark spot at hind margin next to tip of Cu; very faint, yellowish bands from clavus tip to base of Ms1, from apex of Cu to basal cross-vein between M and R, across apical parts of first and second costal cells, along anterior apical cross-veins, and between apical cross-veins and apical margin. Facial carinae contiguous at bases; subantennal processes connected to margins of facial carinae by a ridge. Rostrum reaching post trochanter. Forewings with Ms1 branched very close to apex of basal median cell; base of Ms1a connected to M by a cross-vein; base of Cu1 connected by a short cross-vein to base of Ms1 (lectotype) or to base of Ms1b. Hindwings with tip of R not branched. Male pygofer narrow, slightly produced at the sides. Genital styles oval; proximal dorsal process short and slender; distal dorsal process long. Apical part of aedeagus long, slightly curved down, ending in two rounded, contiguous lobes; on the right side, an erect, pointed dorsal process.

Etymology. The species name is a Latin participle ( quadramaculatus , -a, -um = with four spots).

Type material. Levu quadramaculata , lectotype ♂ (here designated) (forewing 4.2 mm), TAIWAN; labels 1) TYPE OF / L./ 4-maculata/ Muir 2) Levu / 4-maculata/ 654 Muir ( BPBM #5209); Rhotana shirozui , holotype (not examined) ♂, TAIWAN, Rimogan-Magan, 10. VII.1961, Takashi Shirozu ( EUMJ).

Note. Muir described this species from Taiwan "Arisan". Male and female characters are listed, indicating that his description was based on several specimens. Only one specimen is present in BPBM with corresponding data and bearing a type label which is here designated as lectotype .

Other material examined. 1 ♂, no data ( BPBM) .

Distribution. Taiwan.

Diagnosis. Muiralevu quadramaculatus can be recognized by the prominent four marks on the forewings. Only M. exotivus (Yang & Wu) , also from Taiwan, has similar markings, but is larger and lacks the pointed process on the apical part of the male aedeagus.
