Holotype: BPBM 7279, 25.2 mm SL.

Type locality: reef off northwest side of Cocos Island, Guam, Mariana Islands, western Pacific, depth 29 to 36.5 meters. Ilustrations: Heemstra & Randall, 2009:13, fig. 10; Kuiter, 2004:112, fig. left column mid page; Randall, 1980:160, fig. 22; Williams, et al, , 2013:173, fig. 6.

D: X, 13 to 16. A: III, 6 to 8 (usually 7). P: 14 to 16. C: 13 to 15, branched. V: 26 (10 + 16). S: 2. GR: 4 to 7 + 11 to 14. LL: 15 to 22 (incomplete, with mid-lateral series of pored scales on peduncle). CP: 14.

Distribution: widespread in the Indo–Pacific from the Red Sea to the Hawaiian, Line, and Marquesas islands, and Pitcairn Islands.
