Length: 4.7–6.1 mm. Color as in O. rarus group( Fig. 1b). Vertex rounded, median length about equal to distance between eyes at posterior margins. Pale apical spot with narrow arms extending along anterior margin almost to ocelli, wider arm extending posteriorly, with bright orange between eyes. Ocelli, apical spot and its lateral arms surrounded by brown ( Fig. 6). Narrow white lines along inner margins of eyes ( Fig. 70). Pronotum mottled brown with band of irregular light and dark spots along anterior margin. Scutellum with broad light brown band below center margined laterally with brown on posterior half; large brown spots in basal angles, four small white dots along anterior margin. Anterior wing transparent, largely withish, several cells with elongate brown spots; usually with brown spots along costa and commissural line.

Male genitalia: Pygofer about twice as long as greatest width, shaped almost like a parallelogram, moderately projected, apical portion very short, apex widey rounded and triangular; macrosetae long, few, dispersed along apical third of pygofer ( Fig. 37a). Valve triangular. Plates long, apex rounded, margins with folds and plumose setae ( Fig. 37b). Style with apophysis long and straight ( Fig. 37c). Aedeagus in ventral view wide at base, curved; shaft straight, long, very narrow, apex rounded, shorter than processes; dorsal projection short, straight, apex truncate ( Fig. 37d). Ventral processes arising from midlength of aedeagus, fused basally then divergent, apices pointed but irregular in form, striae slightly visible and line-like. In lateral view, shaft with base wide and curved dorsad, apex rounded; dorsal projection short, straight, truncate, and nearly the same size as the aedeagal shaft. Gonopore apical. Ventral processes close to shaft, tapered gradually, apices pointed ( Fig. 37e).

Diagnosis. The color and markings of the head are distinctive in O. fuentesi , with a defined line, a wide irregular white one, and a very irregular triangle near the vertex. The genitalia are similar to those in O. anonae , but in ventral view the ventral processes of O. fuentesi are completely divergent; in lateral view the processes are longer than the shaft, the aedeagus is curved and the dorsal projection is nearly the same size as the aedeagal shaft. In O. anonae the ventral processes are gradually divergent; in lateral view, the aedeagus slightly curved, and the ventral processes similar in size to the shaft.

Distribution: Known only from Costa Rica; 50–1500 m.

Etymology. This species is named for Gilbert Fuentes, in recognition of his work in compiling Costa Rican entomological literature.
